Two of the below documents are required along with your Government issued photo ID:

• Makeup artistry school certificate from a recognized makeup school*

• Current cosmetology or esthetician license

• Call sheet (with name credit for paid print, film or video job- dated with in the last 12 months)

• Tear sheet (with name credit for paid print, film or video job- dated with in the last 12 months)

• Business website (with portfolio and services)

• IMBD Web Link

• Union Card (with name printed on it)

*Please note that makeup artistry school must be active with website provided. Short-term courses/seminar certificates are not accepted. Certification course must be 80+ hours to qualify.

**Social media, such as Instagram pages, will not serve as a substitute for a business website.
